  • Client's full name and date of birth
  • Diagnosis and reason for referral
  • Relevant funding details (NDIS number, DVA file number, claim number etc.)
  • Client's availability and preferred appointment frequency
  • Contact person for booking (if different from the client)
  • Relevant medical history, reports or assessments

We receive your referral

Our admin team reviews the referral and confirms eligibility and funding within one to two business days.

We contact the client

We reach out directly to the client or their carer to book an initial appointment at a time that suits.

Assessment begins

The clinician completes an initial assessment and develops a care plan tailored to the client's goals.

We keep you in the loop

Progress reports go to the referring practitioner. We can coordinate care with your team as needed.
